How To Activate

  1. Fill out WVA Customer Credit Application.
  2. Sign and date the application.
  3. Return the application via fax to (800) 298-2329.
  4. Upon approval of your application WVA will contact you and provide your new WVA Buying Group Account Number.

This account will also be used for all of your contact lens purchases.
